Each state can have its own rules to facilitate implementation of RTI Act.  Given below is the so called Form A according to the Karnataka Right to Information Rules, 2005.

Rule 4 talks about the Form A but it is ok to follow another format of RTI application as long it has the relevant information:

4. Fee.- (1) Any person desirous of obtaining information under sub-section (1) of section 6 of the Act shall make an application in Form-A or in any other format as far as possible containing the particulars specified under the format to the State Public Information Officer or State Assistant Public Information Officer as the case may be along with an initial fee of Rs. 10 with his application. Every officer receiving request under the Act shall give an acknowledgement.

Following is the same template along with information on how to frame questions etc.

i.        Keep it short. Karnataka RTI rules prescribe limit of 150 words.
ii.      Application should ask information about one subject matter, ask separate category of information in separate RTIs. This is as per Rule 14 in Karnataka RTI rules, but even otherwise it is advisable to track your RTI applications better.
iii.    Do not frame questions like “Why xxx is not done”…It will be rejected as this is not information as defined under RTI act.
iv.    Questions can be framed like “How much is the budget …”, “When xxx is planned to be done”, “When xxx will be done”, “Give name, designation of officer responsible for xxx”, “Give the details of steps taken on xxx”.
v.      You do not need to give reasons for asking the information.
vi.    You may give supporting information as enclosures like copy of newspaper story, picture etc

LIC Bangalore has released the Gratuity cheque to my previous company, i left the organisation last month.

I need to get the information from LIC for the gratuity settlement.

I want to know the status of the OC (Occupancy Certificate) for my apartment, the apartment comes under Anekal Planning Authority (APA) under BMRDA limit, but I couldn't find the PIO officer for APA.
Do I need to file the RTI against the BMRDA PIO directly?

Need your assistance on this.

This is the first time I have visited the blog and hats off to Vivek for his efforts to educate and inspire people to use this momentous legislation to solve their day to day problems that needs to be solved by the Government.

Vivek, the roads and drains in my neighbourhood need urgent repairs but there is no response from the recalcitrant BBMP officials. How can I make use of the RTI act to solve this problem?

BBMP is covered under RTI... you need to find out the PIO for roads and drains dept (they may be different) from BBMP website.  File an RTI.
Don't forget to file first appeal if there is no response to RTI within 30 days.
